- Statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
- Signature of the authorized person that acts on behalf of the owner of an exclusive right that is allegedly being infringed.
If you don't contact us directly but rather contact out Internet Service Provider, Domain Name Server hosting company or website hosting company this will only prolong the time needed to meet your claim and result in problems for every party including yours.
